Standard mattresses are covered in quilted fabric. This is lovely for a nap, but it is a disaster for a scene. Once a liquid (of any variety) soaks into those internal layers of foam or felt, it stays there. You can’t exactly put a King-size mattress in the wash, can’t you?
The Mold Factor: Dampness inside a mattress leads to mold. You do not want your submissive breathing in spores while they are pinned down.
The Scent of Shame: Dried fluids have a "fragrance" that is very hard to mask. If your play space smells like a locker room, you’ve lost your edge.

I am not talking about those crinkly plastic covers from the local department store that sound like a bag of chips every time someone shifts their weight. That’s for toddlers. You want high-performance protection.
PU-Coated Protectors: Look for Polyurethane (PU) backing. It is breathable but 100% waterproof. It won't make your submissive sweat excessively (unless that’s your intention), and it’s silent.

High-Density Polyfoam: If you go the foam route, look for a high-density "support core." Lower-density foams will "bottom out" when you apply concentrated pressure (like a knee to the chest or a heavy weight).
Edge Support: This is non-negotiable. If you are tying someone to the side of the bed, or if you like to sit on the edge while they are on the floor, the perimeter needs to be reinforced. Cheap mattresses collapse at the edges.

Use a "Sacrificial" Layer: Even over your waterproof protector, I recommend a heavy-duty, dark-colored top sheet or a specialized "sex blanket." These are designed to be thrown in a hot wash and can take the abuse of rope friction so your mattress doesn't have to.
Avoid Light Colors: This should be obvious, but I’ve seen beginners try to run a scene on white linen. One drop of red wine, candle wax, or... other things, and that set is ruined. Stick to blacks, deep purples, or greys.
Nothing says "I am in control" like a perfectly clean, sterilized environment. After every session, strip the bed. If you have a waterproof cover, spray it down with a body-safe disinfectant and wipe it dry.
